How should an office record the promotion pay-fixation option and effective date?

An employee promoted shortly before the normal increment date may choose between fixation from the promotion date and fixation from the next increment date. What sequence of option, undertaking, verification and office-order entries has worked well in practice? The case has been generalized and contains no employee or office identifiers.

Use a dated option form that identifies both permissible fixation paths in plain language. Record the date of receipt, verify that it falls within the allowed option period, and quote the accepted option in the fixation statement and the speaking portion of the office order. Keep the calculation sheet and option together so a later audit can reconstruct the decision.

Why

The control is less about the form's design than proving that the employee made an informed, timely choice and that the sanctioned fixation follows that choice consistently.
