What should the evaluation note record when the lowest bidder fails a mandatory criterion?

A bid ranked lowest on price does not meet one mandatory technical requirement stated in the tender. Before the next eligible bid is considered, what should the committee record so the decision is transparent, reviewable and procedurally fair?
